Electronic questionnaires and reports submitted to the Regulatory Authority for Electronic Communications and Postal Services are PDF files with activated fields for entering information, which have been sent electronically to the Agency [NP1] via email or Online Desk since 2012.

1.      The Regulator collects the following forms in the described manner:

3.      Annual reports on quality parameters (Forms PPK)

4.      Questionnaires for providing data relevant to determining the amount of fees for performing electronic communications activities (Form IFN)

5.      Questionnaire for providing data relevant to determining the amount of fees for numbering (Form NUM)
